Today’s case is a 15 year old Cairn Terrier with weekly episodes of syncope. Take a look and post your comments!
There is marked right heart enlargement. The caudal border of the heart is straightened. There is a moderate bulge of the mainstem pulmonary artery and distension of all pulmonary arteries. A marked, diffuse interstitial pattern is present. Within the viewable abdomen the liver is severely enlarged.
Pulmonary hypertension (confirmed) secondary to pulmonary fibrosis (presumed)
An echocardiogram was performed.
Subjective: The left atrium and left ventricle are normal in size. There is no evidence of mitral valve regurgitation. There is no evidence of aortic insufficiency. The right atrium is normal in size. The right ventricle is moderately hypertrophied and moderately dilated. There is diastolic and systolic interventricular septal flattening. Tricuspid valve regurgitation is present and estimates the pulmonary artery pressure to be 108mmHg.
Assessment: Severe pulmonary hypertension. Pulmonary hypertension is most likely secondary to lung disease such as pulmonary fibrosis, infection, or chronic bronchitis.
