Today’s case is a 6 year old female neutered Corgi with weight loss and enlarged popliteal lymph nodes.
The cardiac silhouette is normal in size and shape. There is a diffuse interstitial pattern throughout the lungs. There is a mass effect dorsal to the cardiac silhouette with ventral displacement of the mainstem bronchi. The cranial mediastinum is widened, and there is a broad based soft tissue opacity dorsal to the second sternebra. In the portion of the abdomen included, the liver is enlarged and rounded.
Sternal and hilar lymphadenopathy, hepatomegaly, and interstitial pattern are most consistent with lymphoma. Inflammatory disease is less likely.
B-cell lymphoma was diagnosed with fine needle aspirates and immunophenotyping.
Charlierak says
There is a soft tissue opacity dorsal to second and third sternum. The thoracic trachea is normal in diameter and structure but the mainstem bronchus are rather compressed by the round soft tissue opacity. There is diffuse unstructured interstitial lung pattern and more severe in the craninal lungn lobe on DV view. The angle between both mainstem bronchus on DV projection seems mildly widened. The ventral liver lobe is rounded and out of the rib cage with normal serosal detail in abdomen.
1. Two soft tissue masses in the cranial ventral mediastinum and the carina
2. Diffuse interstitial unstructured lung pattern.
3. Moderate Hepatomegaly.
DDX
Mass – lymphoma most likely consistent with enlargement of the popliteal lymph nodes. Thymoma, Abscess and granuloma are unlikely.
Lung pattern – Neoplasia(diffuse infiltrative neoplasia), Edema – Noncardiogenic edema. Hemorrhage and pneumonia is less likely due to lack of the clinical symptoms.
Hepatomegaly – Neoplasia(primary or secondary to lymphoma, mast cell, histiocytoma), metabolic, vascular reasons.
