Today’s case is an 11 year old male neutered French Bulldog with otitis media. What are your findings?
The cardiac silhouette is normal in size. There is increased opacity at the heart base causing widening of the cranial mediastinum on the dorsoventral projection. The pulmonary parenchyma is normal. There are multiple vertebral anomalies consistent with the breed.
Charlierak says
On lat view, There is thoracic verterbal malformation especially T6 and T7, consistent with breed specific changes. Cranial lung lobe is retracted from the ventral thoracic wall with increased soft tissue opacity in pleural space. This seems fat tissue. The ventral tracheal cartilage is mineralized, which is a senile change. Sternal contact of the cranial cardiac silhouette increased.with cardiomegaly on VHS scale but need to take futher consideration of breed characters and obesity.
On DV view, the cranial mediastinum is markedly widened with increased soft tissue opacity displacing the trachea to the right. The left mainstem bronchus is sharply deviated by the soft tissue opacity. The cardiac silhouette looks enlarged but no concurrent pulmonary vessels changes happens on all views.
Taking mediastinal mass into cosideration, Cardiomegaly without discernible chamber enlargements, clinical symptoms of HF and pulmonary vessel changes increases the possiblity of neoplasia such as chemodectoma and hemagiosarcoma around the base of the heart.
Mediastinal mass deviating the trachea – Cranial mediastinal lymphadenopathy but less likely.
Interstitial pattern without clinical symptoms may be caused by the small thorax cavity and obesity.
